Please note: Once you complete all the required information to receive your application kit and select
your delivery option (e-sign, download and print, or have Chase mail the documents to you), you will need to complete a self-certification form.
The self-certification form must be signed and returned to Chase along with the Chase
Select Private Student Loan application/promissory note and other requested documentation.
If your loan application is approved, you will receive a Loan Approval Disclosure Form; and, if you want to move forward with the loan process,
you must read and accept the terms presented in the Loan Approval Disclosure Form within the
time frame given. Prior to disbursement, you will also receive a Loan Final Disclosure Form.
